Authors will need to opt-in to receive offprints of their articles/notes/essays. When you have accepted a piece for publication and have received the signed license agreement back, you should then send them the link to your Journal's Google form where they can select how many complimentary offprints and issues they want. The form also contains information on how to order additional offprints (at cost) directly from the printer.

Please make sure when you are accessing the sheet that you are making changes only to your Journal's tab.
To keep the sheet manageable and easy to use, please clean up past issues' author's responses as necessary.
The responses from the separate forms will go into one master spreadsheet for ease of use, which is linked here. You will use the information in this sheet to complete the author offprint form that you will send to Christensen when you submit your files for Contract Proof preparation. (See below for a blank file and an example file.)
